Community Partnership on Aging

The Community Partnership on Aging (formerly known as Tri-City Consortium on Aging) was established in 1978 by the cities of South Euclid, Lyndhurst and Highland Heights. Its mission is to promote independence, a healthy lifestyle, community involvement and opportunities for personal growth to residents age 60 and older. Mayfield Hts. joined the Partnership in 2011 and Mayfield Village will begin using the services of CPA on May 1. Donna Heath can continue to be reached at the Mayfield Village Senior Services Office at 440.919.2332 for information relating to activities, programs, classes, and trips offered through Mayfield Village Senior Services. The following additional programs and services will be available to Village residents age 60 and over as of May 1st, 2012:

Daily Luncheons:

A CONGREGATE MEAL PROGRAM is offered weekdays. Meals are delivered to the community center where programming is located for the day. Meals are currently offered at the community centers in South Euclid, Lyndhurst, and Highland Heights. Any older adult, regardless of their city of residence, can participate in the meal program. Donations of $1.50 are encouraged. Transportation is available to registered riders. Call 216.650.4029 for questions or to make meal reservations.

Homemaker Services:

HOMEMAKER STAFF employed by the Community Partnership on Aging provide assistance to those unable to perform routine housekeeping tasks. Priority is given to those who do not have others who can assist them, who have health issues, or who are unable to pay for private services. Service rates are $10 per hour and can include linen changing, dusting, vacuuming and other light housekeeping tasks.

Transportation:

TRANSPORTATION is available for medical appointments, errands, to and from the lunch site, and other locations. Vans are wheelchair accessible. Two weekly round trips can be taken for $3 each way to appointments or $2 each way for group shopping. To schedule an appointment to register for the service call 216.291.3902 or 216.548.8010.

Outreach Services:

LICENSED SOCIAL WORKERS provide in-home and office visits to people 60 and older who may benefit from assistance, or who have questions or concerns about matters affecting their physical, social, financial, or emotional wellbeing or independence. Information is available on many local services such as Meals-on-Wheels, support groups, housing options and repairs, legal services, assistance accessing benefits, arranging care, health insurance and Medicare assistance, utility and property tax discounts and voter registration. Social Programs:

A variety of activities are offered, including lunch, discussion groups, exercise classes, art classes, cards and other games, movies, entertainment and lectures. Programs and activities are scheduled Monday through Friday, with lunch served at 11:45 AM (refer to Congregate Meal on reverse). The lunch and programming is offered on a rotating basis at the community centers of cities of South Euclid, Lyndhurst and Highland Heights. Tours and Trips:

Many trips are planned each year for those ages 55 years and older. The trips are planned to provide comfortable travel to exciting and educational destinations, along with companionship and lasting memories. For more trip information, contact the Lyndhurst office at 440.473.5138. Trips will continue to be offered through Mayfield Village Parks & Recreation and Mayfield Village Senior Services.

Wellness and Education:

Whether you enjoy exercise, the creative arts, or health education, The Community Partnership on Aging offers something that will interest you. On-site blood pressure testing, exercise classes, a podiatry clinic and classes to help maintain balance and live with chronic health conditions are available. Other health screenings are offered periodically. Speakers and educators from local health care and college communities also visit to present current topics to meet a variety of interests.

Chore Information:

The Community Partnership on Aging maintains a list of skilled chore people. The list is compiled from references and referrals from other older adults so that you can feel more comfortable using services.
